1500cms.com templates allow users to Filled, Save, Print, and Edited as needed. Each new patient can be saved individually. Use with Adobe Reader for best results. Other PDF readers do not allow users to Save data.

Templates are tested and work with both Mac and Windows using Adobe Reader Version XI and DC, (users must have the current version) get the current version free at http://get.adobe.com/reader/

Need 4 spaces for #3, date of birth, for Medicare, not 2. Any HELP!

Chuck 8 years ago updated 8 years ago 2

Medicare requires 4 digits for the tear in date of birth, 2016, not just 16. Most Ins companies are fine with just the 2. Any way to make this form allow 4 digits or 2 digits? Seems like there should be an easy fix for some cms1500 forms have space for 4 digits for that very Medicare reason. Please help!

I can only find the 1500 forms available for laser printers. Will these work on inkjet printers as well?

can't edit filled pdf Template

KLL 8 years ago updated by info 8 years ago 2

Unable to edit fields in previously saved files and templates. I can however still fill fields in the initial form save. I'm using the latest version of acrobat on os x. Is this normal? Is there any work around, or way to create my own fillable form templates?

I'm happy to help, Simply typing the provider name/supplier is acceptable for progressing. This will not lock the template for further use. However there is a spot on item 31 to add a digital signature (if you choose) to your 1500cms.com template as needed using Adobe Reader.

Item 31 - Enter the signature of provider of service or supplier, or his/her representative, and either the 6-digit date (MM | DD | YY), 8-digit date (MM | DD | CCYY), or alphanumeric date (e.g., January 1, 1998) the form was signed. In the case of a service that is provided incident to the service of a physician or nonphysician practitioner, when the ordering physician or non-physician practitioner is directly supervising the service as in 42 CFR 410.32, the signature of the ordering physician or non-physician practitioner shall be entered in item 31.
When the ordering physician or non-physician practitioner is not supervising the service, then enter the signature of the physician or non-physician practitioner providing the direct supervision in item 31.

A second Clear button

What would be useful is a Clear button that only clears the dates of service section and the date in box 31 and leaves the rest of the form intact. That way you could clear just that particular info while leaving practice, patient and insurance info alone.
