In boxes 32a and in 33b I must put our provider numbers in those boxes. It is required to be approved for the claim through the insurance company. I don't have any issues with boxes 32 and 33 for the address part, only have a problem with the numbers being cut in half in the a and b part.  My printer and print set up are set for A4 page print out and it still cuts it in half.
