I'm happy to help, 1500cms.com 02/12 templates include space for 4 digest. Older versions (08/05 templates) only had space for two digest.

I'm happy to help, Yes, inkjet printers will print on HCFA 1500 forms designed for laser printers.

1500cms.com offers 2 different 1500 forms, "pre filled form" will only print the data that is entered not the entire form, this will print on the standard HCFA claim form. The "print on blank paper" template is programmed to print the entire form, this will print on blank paper and does not require the HCFA paper form (most insurance requires HCFA forms printed with a color printer).


1500cms.com templates will be saved as PDF documents that can be edited each billing cycle, no need to type the entire form again and again.

I'm happy to help, Simply typing the provider name/supplier is acceptable for progressing. This will not lock the template for further use. However there is a spot on item 31 to add a digital signature (if you choose) to your 1500cms.com template as needed using Adobe Reader.


Item 31 - Enter the signature of provider of service or supplier, or his/her representative, and either the 6-digit date (MM | DD | YY), 8-digit date (MM | DD | CCYY), or alphanumeric date (e.g., January 1, 1998) the form was signed. In the case of a service that is provided incident to the service of a physician or nonphysician practitioner, when the ordering physician or non-physician practitioner is directly supervising the service as in 42 CFR 410.32, the signature of the ordering physician or non-physician practitioner shall be entered in item 31.
When the ordering physician or non-physician practitioner is not supervising the service, then enter the signature of the physician or non-physician practitioner providing the direct supervision in item 31.

Use Adobe Reader XI (11.0.10) for best results. The older versions of Reader (8,9,X and Mac Preview ) do not have the ability to save. Make sure you have the latest version of Adobe Reader for best results.

A workaround for now , fill the template with all the information that does not change then "save as" the main template. Then each patient is loaded and "save as" a new document. When ever a new patient needs to be added, simply open the "main" template (with all the information that does not change) and key the individuals missing information.
