
Problem with form. It does not save and print on HICFA paper.
I tordered a new one as it did not transfer to my new computer. I am not able to print on HICFA Forms. It is only printing it's own copy. Please advise.

I'm happy to help, 1500cms.com software is tested to work with Adobe Reader Version XI and DC, (users must have the current version) get the current version free at http://get.adobe.com/reader/ older versions do now work well with 1500cms.com software.
The logo will only display on the screen but will not print, only form fields and HCFA background form will print when using Adobe Reader. Mac "Preview" program, older versions of Adobe Reader and some other PDF readers will not understand the programmed 1500cms.com templates. Always use Adobe Reader for best results.

Need 4 spaces for #3, date of birth, for Medicare, not 2. Any HELP!
Medicare requires 4 digits for the tear in date of birth, 2016, not just 16. Most Ins companies are fine with just the 2. Any way to make this form allow 4 digits or 2 digits? Seems like there should be an easy fix for some cms1500 forms have space for 4 digits for that very Medicare reason. Please help!

I can only find the 1500 forms available for laser printers. Will these work on inkjet printers as well?

can't edit filled pdf Template
Unable to edit fields in previously saved files and templates. I can however still fill fields in the initial form save. I'm using the latest version of acrobat on os x. Is this normal? Is there any work around, or way to create my own fillable form templates?

I'm happy to help, Simply typing the provider name/supplier is acceptable for progressing. This will not lock the template for further use. However there is a spot on item 31 to add a digital signature (if you choose) to your 1500cms.com template as needed using Adobe Reader.
